2. Lock-Out/Tag-Out Principles

Control of Hazardous Energy: Ensuring all sources of hazardous energy (electric, hydraulic, mechanical, thermal, etc.) are properly isolated and de-energized.
3. LOTO Procedures
Preparation: Identify all energy sources that must be isolated and confirm the methods for de-energizing them.
Shutdown: Turn off the machinery or equipment following the manufacturer's procedures.
Lock-Out: Apply lock-out devices to energy-isolating mechanisms (e.g., electrical switches, valves).
Tag-Out: Attach appropriate tags with clear instructions, warnings, and the name of the person responsible.
Verification: Verify that energy sources are effectively isolated and that equipment cannot be started before maintenance begins.
Perform Work: Once energy sources are locked/tagged out, maintenance or servicing can proceed.
4. LOTO Steps for Re-Energization
Complete Work: Before re-energizing, ensure all tools, parts, and personnel are clear of the equipment.
Remove Lock/Tag: Only the person who applied the lock/tag can remove it.
Verify Equipment: Inspect the equipment and ensure that it is safe for operation before re-energizing.
Notify Personnel: Inform all workers that the equipment is being re-energized and they should stay clear.
5. Common Hazards & Prevention
Unexpected Start-Up: Prevent by using proper lock-out/tag-out methods and ensuring that all energy sources are isolated.
Failure to Follow Procedures: Ensure all employees are trained in the lock-out/tag-out procedures to avoid shortcuts that could lead to injuries.
Accidental Removal of Locks or Tags: Ensure that locks are only removed by the person who applied them and that proper communication occurs.
6. Emergency Procedures
Energy Release Accidents: If hazardous energy is released unexpectedly, stop work immediately, secure the area, and report the incident to supervisors.
